Immunization Records (School Module)

According to the Washington Administrative Code WAC 246-105-030 and WAC 246-105-040 any child attending school, preschool, or child care in Washington State is required by law to be fully immunized (documentation of vaccination or proof of acquired immunity) against the following diseases at the ages and intervals in the CDC Advisory Committee on Immunization Practices immunization schedule:

Children entering school, child care, or other early learning programs are required to have certain vaccinations before they can start. These requirements are put into place to protect children, families, and communities from vaccine preventable diseases. Immunization requirements apply to all enrolled children, including those learning remotely.

The parent or guardian of a child must provide documentation of vaccination or proof of immunity to specific diseases. This requirement is based on the national Advisory Committee on Immunization Practices (ACIP) Immunization Schedule for school or child care registration and attendance. The specific diseases are listed below: 

  • Chickenpox (Varicella) 
    • One dose at each of the following ages: 
      • 1st dose: 12-15 months 
      • 2nd dose: 4-6 years 
  • Diphtheria 
    • One dose of DTaP at each of the following ages: 
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6 months 
      • 4th dose: 15-18 months 
      • 5th dose: 4-6 years 
  • Hepatitis B 
    • One dose at each of the following ages: 
      • 1st dose: shortly after birth 
      • 2nd dose: 1-2 months 
      • 3rd dose: 6-18 months 
  • HIB (Haemophilus influenzae type B)
    • One dose at each of the following ages: 
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6 months for some brands 
      • 4th dose: 12-15 months 
  • Measles 
    • One dose at each of the following ages: 
      • 1st dose: 12-15 months 
      • 2nd dose: 4-6 years 
  • Mumps 
    • One dose at each of the following ages:  
      • 1st dose: 12-15 months 
      • 2nd dose: 4-6 years 
  • Pneumococcal disease
    • One dose at each of the following ages: 
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6 months 
      • 4th dose: 12-15 months 
  • Polio (Poliomyelitis) 
    • One dose at each of the following ages: 
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6-18 months 
      • 4th dose: 4-6 years 
  • Rubella (German Measles) 
    • One dose at each of the following ages: 
      • 1st does: 12-15 months 
      • 2nd dose: 4-6 years 
  • Tetanus 
    • One dose of DTaP at each of the following ages:  
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6 months 
      • 4th dose: 15-18 months 
      • 5th dose: 4-6 years 
    • One dose of Tdap at the following ages: 
      • 11-12 year 
  • Whooping Cough (Pertussis) 
    • One dose of DTaP at each of the following ages: 
      • 1st dose: 2 months 
      • 2nd dose: 4 months 
      • 3rd dose: 6 months 
      • 4th dose: 15-18 months 
      • 5th dose: 4-6 years 
    • One dose of Tdap at the following ages: 
      • 11-12 years

*required only until 5 years of age
