Registration for the 2012 PNW AP* Institute has begun.

Option #1 - Fillable PDF (Form may take a moment to load. Option #1 allows you to type and submit by e-mail---Or type, print out and submit by fax or mail---Or print out, complete the form and submit by fax or mail.)

Fill in the form online and click the Submit by Email button. You can only submit by email if paying by credit card or scanning and attaching your purchase order (PO) to your registration e-mail. Please do not submit if waiting on a check or purchase order as we do not "hold" slots.

You must have Acrobat Reader to use this option. Click the icon to download it for free.
Fill in the form on-line and click the Print Form button. You may also click the Print Form button and fill it in my hand. Then fax (425-456-4176) or mail (API, PO Box 90010, Bellevue, WA 98009) the form with your payment.

Option #2 - Fillable Word Form (Type and print out or type and e-mail only)

You can fill in this form and print it, fax it (425-456-4176), mail it (API, PO Box 90010, Bellevue, WA 98009) or e-mail it (save and send option) with payment. *Please note since this is a fillable form the spaces are limited. If you need more space or reside out of the U.S. you might prefer option #1 or #3. Please total your own registration amount.

Option #3 - Plain Word Document (Print out and complete)

You can print this form, fill it in by hand and fax it (425-456-4176) or mail it (API, PO Box 90010, Bellevue, WA 98009) with payment.

Please Note

Make checks payable to: Bellevue School District Advanced Placement Institute (BSD API).

Payment or a purchase order must be included before registration will be processed. Therefore, forms with only a PO number or an incorrect amount will not be processed until a copy of the PO or a corrected PO has been sent. All registration information must be completed. Missing or unreadable information will delay registration.

If you do not receive an email confirmation of your registration within a week, please contact us at api@bsd405.org or 1-877-456-4399. We may not have received your registration form.
