Early Learning Program Information & Forms
Illness Credit Form:
Credit is given only in the event of an absence for serious medical reasons. Requests must be submitted in writing within one moth of the child's absence. Credit is available for a medical absence lasting at least 5 consecutive days of the child's program at the rate of 1/2 day charge for each day absence.
