There are 3 ways to send your registration form:
- Fill in the form on-line* and click the Submit by Email button. (The format will change.) If the registration form is not being emailed by the participant, please state the participant's name in the email. You can only submit by email if paying by credit card or scanning your purchase order. Do not submit if waiting on a check or purchase order as we do not "hold" slots.
- Fill in the form on-line* and click the Print Form button. Then fax or mail the form.
- Click the Print Form button and fill in legibly by hand. Then fax or mail the form.
Payment or a purchase order copy must be included before registration will be processed. Forms with only a po number will not be processed until a copy of the po has been sent.
Please note: ALL information is vital, please completely fill out the form.
Missing or unreadable information will delay registration.
