There are several ways to complete and send your registration form:
- Fill in the form on-line and click the Submit by Email button. (The format will change.) If the registration form is not being emailed by the participant, please state the participant's name in the email. You can only submit by email if paying by credit card or scanning and attaching your purchase order (PO) to your registration e-mail. Do not submit if waiting on a check or purchase order as we do not "hold" slots.
- Fill in the form on-line and click the Print Form button. Then fax or mail the form with your payment.
- Click the Print Form button and fill in legibly by hand. Then fax or mail the form with your payment.
- You may click here for a Word version of the document you can print out, fill in by hand and fax or mail with payment.
- Email api@bsd405.org to ask us to email, fax, or mail you a copy of the form/brochure.
Mac users may need to choose #3, #4 or #5.
Payment or a purchase order copy must be included before registration will be processed. Forms with only a PO number will not be processed until a copy of the PO has been sent.
